Thank you for your interest in a Parkinson Support Grant from the National Parkinson Foundation Minnesota. NPFM is excited to offer three opportunities for Parkinson’s patients and their caregivers through the program, focusing on the areas of caregiver respite care, help-at-home and exercise.

Individuals/families are welcome to apply for ONE of the three grants within a 12-month period. Applications will be accepted on a rolling basis until funds are exhausted, and will be handled in the order they are received to the NPFM office.

Below is a brief description of the three grant programs, and application materials are also included. Although financial information is not required, it is understood that those requesting one of the three grants has a financial need to pay for the service requested.

Before considering which grant is best for you, read the information below, including the guidelines for each grant.

A Diagnosis Statement is required with all grant applications. The form must be completed by the physician of the person with Parkinson’s or a nurse or social worker from the clinic.

Caregiver respite care grant: This grant is for the live-in caregiver of a person with Parkinson’s or another related movement disorder living in Minnesota or one of its bordering U.S. states (North Dakota, South Dakota, Iowa and Wisconsin), allowing the caregiver a period of respite, or relief, from everyday responsibilities.

Help-at-home grant: This grant is for a person with Parkinson’s or another related movement disorder living in Minnesota or one of its bordering U.S. states (North Dakota, South Dakota, Iowa and Wisconsin) to assist with general maintenance of a residential dwelling.

Exercise grant: This grant is for a person with Parkinson’s or another related movement disorder living in Minnesota or one of its bordering U.S. states (North Dakota, South Dakota, Iowa and Wisconsin) to help the person with Parkinson’s get active through exercise programs.
