Work-Related Issues

People with Parkinson's who experience trouble seeking Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits may find this form helpful. It was created by the Parkinson's Action Network with the hope that it will be used by both clinicians and patients as a way to document and track a person's symptoms with particular focus on the symptoms that are most likely to interfere with one's ability to work. The Parkinson's Disease Work-Related Disability Assessment form  is designed to supplement a patient's medical record and is not a Social Security Administration form or application.


Support Groups
The Parkinson Association of MN has a long history of support and interaction with the various support groups of the Midwest Region Support Group Network.  If you would like to find a support group in your area:

National Parkinson Foundation
The Parkinson Association of Minnesota was established as a chapter of the National Parkinson Foundation in January of 1996.  The NPF sponsors research and treatment centers around the world.  If you would like to reach NPF, you may call them at 1-800-327-4545.  They also can be accessed by computer at www.parkinson.org.
 

Capistrant Center for Parkinson's Disease and Movement Disorders
The Capistrant Center at Bethesda Hospital provides medical evaluations, treatment, support and resources for those with Parkinson's Disease and Movement Disorders.


Struthers Parkinson's Center
The Struthers Parkinson's Center is an NPF Center of Excellence.  It offers clinical, research, educational, therapeutic, and artistic programs for persons with Parkinson's throughout the Upper Midwest.
